Before you begin, make sure to have your dental and medical insurance information ready, along with the contact information of your family dentist and primary care physician. You may also want to have a list of your prescription medications.

When you are ready to begin, click the “Online Registration Form” button below. After you have completed the form, the patient information collected is encrypted and sent securely to our office. The registration process will take you about 20 minutes.
