This narrated interview system enables a more accurate form completion process. This presentation will save you time! It eliminates the need for you to fill out traditional healthcare forms in our office.

After you have completed the interview, the patient information collected is securely encrypted and sent to our office. Online registration is not mandatory. You are welcome to fill out the forms in the comfort of our waiting room. Or, if time allows, you may ask us to mail the forms to you which will allow for their completion in the comfort of your home.
TO ACCURATELY COMPLETE THE ONLINE REGISTRATION FORMS, FIRST HAVE YOUR DENTAL AND MEDICAL INSURANCE INFORMATION CLOSE AT HAND, ALONG WITH THE CONTACT INFORMATION FOR YOUR FAMILY DENTIST AND PHYSICIAN. YOU MAY ALSO WANT TO HAVE A LIST OF YOUR PRESCRIPTION MEDICATIONS. AS YOU BEGIN YOU WILL BE ASSIGNED A PIN NUMBER. WRITE THIS NUMBER DOWN! YOU WILL NEED IT TO GET BACK TO YOUR PAGE IF YOU NEED TO STOP THE REGISTRATION FOR ANY REASON.
